PROGNOSTIC VALUE OF EXERCISE RIGHT VENTRICULAR FREE WALL STRAIN IN PATIENTS WITH SICKLE CELL DISEASE

BackgroundExercise stress echocardiography (SE) is increasingly recognized as a useful tool for assessment of pulmonary hypertension (PH) in patients with sickle cell disease (SCD). However, while the prognostic significance resting systolic artery pressure (SPAP) well established and global longitudinal right ventricular free wall strain (RV FWS) has been identified an independent marker PH, little known about value RV FWS this population, whether SPAP elevated at rest or not. We therefore examined response to exercise SCD normal SPAP.Methods ResultsConsecutive referred our center bicycle ergometer SE measurable tricuspid regurgitation velocity (TRV) were enrolled from July 2019 January 2021. Conventional parameters, including TRV estimated SPAP, left (LV GLS) FWS, assessed peak exercise. Exercise performance was also recorded. Repeat then performed median follow-up 2 years. The cohort consisted 96 (mean age 30±11 years, 68% females), all whom had (TRVrest) < 2.8 m/s, LV GLS baseline. There 74 (77%) who (TRVpeak) > m/s. They higher E/e’, atrial volume index basal diameter compared TRVpeak ≤ 2.8m/s (p 0.05 all). Resting did not differ between both groups, but worse among those m/s (-23±4% vs. -27±4%, p=0.001). At baseline experienced non-significant worsening performance.
